Melatonin sits on the same shelf as vitamin C and fish oil, sold as gummies in flavors aimed at both adults and children, marketed as gentle and natural because the body makes it anyway. That framing makes an important question easy to skip past: is taking it every night, for months or years, actually established as safe?
The short answer: this is more unsettled than the over-the-counter packaging suggests. Melatonin has a reassuring short-term safety profile in the doses studied, but it’s a hormone, not a vitamin, most trials only run for weeks, commercial products routinely contain far more than the doses shown to work, and label accuracy has been a documented problem. None of that means it’s dangerous — but “safe long-term” hasn’t been demonstrated the way the casual marketing implies.
It’s a hormone, regulated like a supplement
Melatonin is produced naturally by the pineal gland in response to darkness, and it’s part of the signaling that tells your body it’s nighttime. That’s a meaningfully different category from a vitamin your body can’t synthesize at all. In the United States, melatonin is sold as a dietary supplement rather than a drug, which means it doesn’t go through the same premarket safety and efficacy review the FDA requires for medications. Several other countries regulate it as a prescription product for exactly this reason. The looser US regulatory category is a big part of why the question of long-term safety is less thoroughly answered than for most sleep medications.
Commercial doses are often far higher than what’s been studied
The doses used in clinical research on sleep onset are typically small, often well under a milligram to a few milligrams. Many commercial products, particularly gummies, are sold at 5 or 10 milligrams per serving, sometimes more — doses considerably higher than what trials have used to show an effect, without good evidence that more produces a better result. Higher doses are more associated with next-day grogginess and a “melatonin hangover” feeling than with better sleep. If a product’s effective dose and its marketed dose have drifted apart, that’s worth knowing before assuming more must be better.
Label accuracy has been a real, documented problem
Because melatonin is regulated as a supplement rather than a drug, manufacturing isn’t held to the same testing standards. Independent lab analyses of commercially available melatonin products have repeatedly found that the actual content can differ substantially from what’s printed on the label — sometimes containing much less than claimed, sometimes considerably more, and in some cases detectable amounts of unlisted compounds like serotonin. That’s a quality-control issue as much as a melatonin-specific one, but it means the “3 mg” on a bottle isn’t a guarantee, and it matters more for a hormone taken nightly over a long stretch than for an occasional dose.
What the long-term data can and can’t tell you
Most randomized controlled trials of melatonin are short, typically weeks to a few months, and conducted in adults. That body of research generally hasn’t turned up serious safety signals at the low-to-moderate doses studied. But the absence of alarming findings in short trials is not the same as proof of safety across years of nightly use — that kind of long-duration data is much thinner. Researchers have also raised specific questions about chronic use in children and adolescents, since melatonin is involved in the timing of puberty, and that population has been studied even less than adults. This is an area of open caution rather than a settled verdict either way.

What a more cautious approach looks like
If you’re going to use melatonin, the lowest dose that helps is a reasonable starting principle, since trials generally used low doses and higher amounts haven’t shown a clear benefit. It also tends to be studied and used most confidently for circumstantial, time-limited situations — jet lag, adjusting to a new shift schedule, a temporarily disrupted routine — rather than as an indefinite nightly habit. If you find yourself relying on it every night for months to fall asleep, that’s less a sign melatonin needs a higher dose and more a signal worth raising with a clinician: chronic difficulty falling or staying asleep can have an underlying cause, from a treatable sleep disorder to a medication interaction, that a supplement will only ever mask.
Melatonin also interacts with certain medications, including some blood thinners, immune-suppressing drugs, and diabetes medications, and isn’t recommended for children, or during pregnancy or breastfeeding, without medical guidance.
