Is "No Pain, No Gain" a Useful Rule for Exercise?

“No pain, no gain” has been shouted across gym floors and aerobics studios for decades, and it still shows up as an unofficial rule of thumb: if a workout doesn’t hurt, it isn’t working. But the phrase asks a real question worth answering carefully — does progress actually require pain, and if so, what kind?
The short answer: the rule mixes up two very different sensations. The burn and soreness of genuine training effort are a normal, often necessary part of getting stronger. Sharp, sudden, or joint-level pain is not — it’s a signal to stop, not a badge of honor. Confusing the two is where “no pain, no gain” goes from motivating to genuinely risky.
Where the phrase came from
The slogan is closely tied to the fitness boom of the 1980s, when aerobics classes and home workout videos popularized language like “feel the burn” alongside a broader gym-culture ethos that treated visible suffering as proof a workout counted. Bodybuilding circles had long used similar mantras to describe pushing through the final, hardest reps of a set. The phrase caught on because it’s simple and stirring — but simple slogans tend to flatten nuance, and this one flattened an important distinction.
Two different feelings get lumped together
Exercise physiologists generally separate “training discomfort” from “injury pain,” and the difference matters more than the shared word suggests.
Effort and fatigue. The burning sensation during a tough set of squats or a hard interval comes from working muscles under load — a normal, expected part of pushing your current capacity. Delayed-onset muscle soreness (DOMS), the stiffness that shows up a day or two after a new or harder-than-usual workout, comes from small-scale muscle strain that the body repairs and adapts to over time. Both are uncomfortable. Neither is damage in the sense most people picture.
Sharp, localized, or joint pain. A stabbing sensation in a joint, a sudden pull, pain that worsens as a set continues rather than easing into it, or pain that shows up during a specific movement and not others — these point toward actual tissue stress or injury, not routine training adaptation. This is the kind of pain the body is using as an alarm, and pushing through it is where “no pain, no gain” stops being motivational and starts being a liability.
What the research on soreness actually shows
A common assumption is that more soreness means a more effective workout — that if you’re not sore the next day, you didn’t try hard enough. Research comparing training approaches has generally not supported that idea. Soreness and strength or muscle gains don’t track together reliably: people following very similar strength programs can end up with comparable results despite very different amounts of next-day soreness, and some of the most productive training sessions leave people barely sore at all.
That matters because chasing soreness as a goal in itself can backfire. Training so hard that you’re too sore to move well for days tends to interrupt consistency, and consistency — training regularly over months, not any single brutal session — is one of the more reliably important factors in long-term progress.

When “push through it” turns risky
The riskiest version of “no pain, no gain” is using it as a reason to override the body’s warning system. A few patterns are worth taking seriously rather than powering past:
- Pain that gets worse as a workout continues, instead of loosening up after a few minutes.
- Pain localized to a single joint, tendon, or one side of the body, rather than the general fatigue of working muscles.
- Pain accompanied by swelling, clicking with instability, numbness, or a noticeable loss of strength or range of motion.
- Pain that lingers well beyond the normal one-to-three-day window of ordinary post-workout soreness.
None of that is a reason to avoid exercise altogether — it’s a reason to back off the specific movement, adjust load or form, and get it evaluated if it doesn’t resolve. A physical therapist, sports medicine physician, or qualified coach can help sort out whether something is routine training stress or a developing problem, which isn’t always obvious from the inside.
So is the rule completely wrong?
Not entirely, and that’s part of why it has stuck around. There is a real, defensible version of “no pain, no gain”: meaningful strength and endurance gains do generally require training hard enough to feel genuine effort and fatigue, not just going through the motions comfortably. Progressive overload — gradually asking your muscles to do a bit more over time — is a well-supported principle of getting stronger, and it does not feel effortless.
The problem is the word “pain” itself, borrowed for its punch rather than its precision. What actually drives adaptation is effort, progressive load, and recovery, not suffering for its own sake, and definitely not pain in the medical sense of tissue-level injury signals.
The bottom line
“No pain, no gain” survives because it captures something true in an imprecise way: real progress requires real effort, and comfortable, easy workouts alone tend not to produce much change. But the phrase’s literal claim — that pain itself is the mechanism, and more of it is better — doesn’t hold up. Muscle fatigue and next-day soreness are a normal part of training hard; sharp, joint-level, or worsening pain is a different thing entirely and should stop you, not spur you on. Train hard enough to feel it. Just don’t mistake an injury warning for encouragement.
If a specific pain doesn’t ease within a few days, keeps returning with the same movement, or comes with swelling or instability, it’s worth having a physical therapist or physician take a look rather than training through it.
